This information is required by the government and will only be used for the purposes of easily contacting you in the event that there is a confirmed case of COVID-19 at a Kingswim centre.
All information will be deleted after 28 days.
If you are the primary account holder, you do not need to complete this form as we will already have your contact information.
Where a guardian is accompanying multiple children for lessons, only the name of one child is needed for this form.
